That’s what an incredible doctor told me when I first started navigating the American healthcare system years ago.
As a Brazilian, I naturally relied on the national healthcare system. But my professional experiences led me to explore healthcare systems across Latin America, including Mexico, Chile, and Peru. I also lived in Barcelona, where I experienced CatSalut, Catalonia’s public healthcare system, which provides universal coverage through a mix of public and private providers. Later, I moved to the UK and navigated the NHS, the publicly funded system that offers comprehensive care to all residents.
But something felt different in the U.S. – and after speaking with dozens of American physicians, I have to say that this mentor doctor was absolutely right.
How do you even begin to explain that:
- Primary care doctors working within the system often manage panels of over 2,000 patients, conduct 20–30 appointments a day, and are constantly overwhelmed with administrative tasks – all while using technology they have no choice over.
- Patients rarely have clarity on how much they will pay for appointments, lab tests, or procedures when using their insurance, only to receive costly surprise bills ($$$) at home. It’s no surprise that medical debt is one of the leading causes of financial hardship.
However, there are some incredible people with real attitude and dedication working to improve how patient care is delivered in the US. They offer much more time for each patient and focus on comprehensive and preventive care. To name it, I'm talking about Direct Primary Care.

To be more precise, I’ll use Dr. Garrison Bliss’s definition, in which he explains that Direct Care is an intentional culture built on a commitment: doctors work exclusively for their patients, ignoring or avoiding all other influences.
